Job summary

Midlands Partnership NHS Foundation Trust is seeking an Outreach Support Worker (Band 4) to assist vulnerable adults in the community who have involvement with the criminal justice system. The role focuses on reducing reoffending and addressing health needs through up to 12 weeks of support, including assisting with appointments and access to services.

Qualifications

• NVQ Level 3 Health & Social Care or equivalent experience.
• Police Vetting and/or Prison Clearance required.
• Experience with a range of vulnerabilities (mental/physical health, learning disabilities, addiction, social issues).
• Experience with a range of statutory/non-statutory agencies.
• Experience in the Criminal Justice System and community settings.

Responsibilities

• Independently undertake face-to-face and electronic screening to identify vulnerabilities in individuals involved with the criminal justice system or immigration centres.
• Support qualified staff and other workers as needed.
• Provide an embedded service in custody suites, courts and prisons within relevant legislation.
• Work with service users and professionals to develop and implement individualized support plans.
• Liaise with other agencies and professionals to coordinate care.
