Job summary

West Suffolk NHS Foundation Trust is looking for a Reablement Support Worker (Development Post) to help patients return home safely after hospital discharge. This role involves delivering a personalised shared care plan and supporting daily living tasks in patients' homes across West Suffolk.

You will travel in the community, hold a full UK driving licence, and participate in a development programme with progression to Band 3 Senior Home Care Support Worker on meeting competencies.

Qualifications

• Level 1 qualification in Maths and English.
• Care Certificate.
• Level II NVQ - Health related or willingness to achieve within first year.
• Experience in a customer/focused environment.
• Experience of caring for others in a positive and respectful manner.

Responsibilities

• Support patients to become more independent with daily living tasks after discharge.
• Assist with personal care, meals, and mobility.
• Travel across West Suffolk to provide care in patients' homes.
• Support and provide palliative care for patients at end of life.
• Work flexible shifts, including weekends; service operates 7 days a week.
• Progress via development programme to Band 3 Senior Home Care Support Worker.
