Job summary

Cornwall Partnership NHS Foundation Trust seeks a Community Support Worker to support reablement and rehabilitation across Penwith communities. You will motivate service users to regain independence and help them return home from hospital when possible.

You will work within an integrated team, receive bespoke training, and develop communication skills for older patients, including those with frailty or communication difficulties. A 7-day service is provided.

Qualifications

• NVQ Level 3 qualification in care or equivalent experience.
• Experience in a care setting is essential.
• GCSE Maths and English (or equivalent).
• Additional relevant healthcare training is desirable.

Responsibilities

• Provide versatile, responsive care to optimise reablement in the community.
• Be part of an Integrated Care Pathway to prevent hospital admission and support discharge.
• Deliver packages of care to patients in their own homes, including care in residential/nursing homes.
• Be flexible with working patterns to meet service needs and rosters.
