Job summary
Care Dorset Ltd. is seeking a Community Reablement Worker to support individuals in the North Dorset area. You will help clients regain independence at home through personalised reablement plans and holistic care, working closely with a multidisciplinary team to ensure high-quality services.

Qualifications
- NVQ Level 2/3 in Health & Social Care or equivalent.
- Experience delivering personal care and reablement.
- Experience assessing client needs.
- Ability to deliver person-centred care.
- Strong communication skills, verbal and written.
- Organisational skills with caseload management.
- Understanding of safeguarding policies and procedures.
Responsibilities
- Deliver reablement care in clients' homes, promoting independence.
- Administer medication as directed by a GP in line with guidelines.
- Develop and review tailored person-centred care plans.
- Conduct initial assessments and regular progress reviews.
- Collaborate with occupational therapists, nurses and social workers.
- Monitor progress and document wellbeing; report concerns.
- Support safeguarding and risk reporting according to policy.
