Job summary

Wokingham Borough Council is seeking a Social Care Practitioner to support adults to live safely, live well, and stay connected in their communities. You will work under supervision from a social worker/occupational therapist and contribute to a compassionate, strengths-based care approach.

This full-time permanent role offers a clear development pathway, with progression to Grade 6 upon meeting criteria, and the opportunity to work within an award-winning council renowned for its social care

Qualifications

• Experience in health or social care is desirable.
• Strong IT skills to use adult social care recording systems.

Responsibilities

• Carry out strengths-based assessments and reviews of people’s day-to-day lives and their support arrangements.
• Gather information and feedback from individuals, carers, professionals and advocates as needed.
• Write reports and keep accurate records.
• Build positive relationships with residents and support a wide range of communication needs.
• Work closely with colleagues as part of a team.
