Job summary

Wokingham Borough Council is seeking a Social Care Practitioner to join our Adult Social Care team. This full-time, permanent role offers the opportunity to support adults to live the lives they choose, stay safe, and live well in their communities, with supervision from a social worker or occupational therapist.

You will focus on strengths-based approaches, build relationships with residents, and work as part of a supportive team.

Qualifications

• Strong values and respectful, trustworthy attitude.
• Good IT skills and ability to use adult social care recording systems.
• Able to engage with people with different communication needs.
• Inclusive approach building relationships with people from diverse backgrounds.
• Experience in health or social care or people-facing role.

Responsibilities

• Work with people to find out what is important to them.
• Carry out strengths-based assessments and reviews of people’s day-to-day lives and their support arrangements.
• Gather information and feedback from individuals, carers, professionals and advocates where needed.
• Write reports and keep accurate records.
• Build positive relationships with residents and support a wide range of communication needs.
• Work closely with colleagues as part of a team.
