Job summary

Staffordshire County Council is hiring Support Facilitators (Home Care) to deliver person-centred reablement in the community. You will join the Provider Services team and work across North/East Staffordshire to support adults in their own homes.

The role requires a full UK driving licence, a flexible weekend availability, and travel between locations. Part-time shifts (7:00–22:00) with paid training are offered, with a focus on quality, safeguarding and independence.

Qualifications

• Experience in social care and community-based support
• Strong communication and relationship-building abilities
• Flexible working patterns including weekends and bank holidays
• Full driving licence and access to a vehicle

Responsibilities

• Provide person-centred reablement care in people’s homes
• Support daily living tasks, personal care and medication administration
• Communicate effectively with individuals, families and colleagues
• Respond flexibly to changing needs and use digital tools to improve delivery
• Maintain records and comply with health and safeguarding standards
• Drive to deliver care across the local area and undertake manual handling
