Job summary

Gateshead Health NHS Foundation Trust is recruiting for a Band 6 Nurse Practitioner to join the Rapid Response Nursing Team on a part-time basis (30 hours/week). The role focuses on urgent community interventions to help people stay safely at home and avoid hospital admissions.

The post requires autonomous clinical skills, strong assessment and decision-making, and a compassionate, patient-centred approach to care delivered in patients' homes and local communities across Gateshead.

Qualifications

• Registered nurse with a nursing degree/diploma and valid PIN.
• Experience with PGDs and delegated clinical skills.
• Full driving licence and ability to travel within Gateshead borough.

Responsibilities

• Assess, plan, implement, and evaluate nursing care in community settings.
• Deliver high-quality nursing care to adults in home/community environments.
• Provide autonomous clinical interventions within sphere of competence.
• Contribute to 24/7 Rapid Response service enabling admission avoidance.
