Job summary

NHS Gateshead is seeking a Band 7 Enhanced Nurse Practitioner to join the Rapid Response Nursing Team in Gateshead. The role focuses on urgent community response to prevent deterioration, manage complex needs, and coordinate care across primary, secondary, and social care interfaces in the Gateshead area.

As an autonomous clinician, you will perform advanced clinical assessments, diagnose, plan treatment, prescribe where appropriate, and lead on high-quality patient-centred care in unscheduled

Qualifications

• Nursing degree/diploma in Adult Nursing
• Non medical prescribing or willingness to work towards
• Valid UK driving license

Responsibilities

• Ensure the right care is delivered at the right time, by the right person.
• Ensure safe, caring, effective, well led and responsive care.
• Role model ICORE values and behaviours.
• Uphold the NMC Professional Code of Conduct at all times.
• Support Rapid Response Team lead to deliver high quality, patient centred care in Gateshead.
• Manage service on a day to day basis with the team lead.
• Act as/supply senior clinical decision maker for the team's Single Point of Access.
• Deliver urgent 2 hour community response with emphasis on patient safety and prioritisation.
• Deliver out of hours palliative care.
• Provide intravenous antibiotics at home.
