Essex Partnership University NHS Foundation Trust in Thundersley is seeking a Band 6 Community Palliative Care Nurse to provide holistic end-of-life care in the community. You will manage a caseload of patients, work with a multidisciplinary team and contribute to reducing unnecessary hospital admissions through advanced care planning.

To provide holistic advice and support to palliative patients in the community. Working autonomously providing palliative care in the community. Symptom management is a key feature of the position. To liaise with other health care professionals and agencies to provide seamless care.
