Job summary

Qualifications
- NMC registration in adult/general nursing.
- Qualified nurse with palliative care knowledge or willing to pursue.
- Experience in community or inpatient palliative care.
- Ability to manage caseload and coordinate care.
Responsibilities
- Assess, plan, coordinate and evaluate palliative care interventions.
- Provide face-to-face visits and responsive service.
- Collaborate with MDT and external partners.
- Ensure patient safety and quality care.
- Lead and support workload management and supervision.
- Participate in multidisciplinary meetings.
