Job summary

The Phyllis Tuckwell Hospice in Guildford is seeking a full-time Rehabilitation Assistant to support high-quality rehabilitative palliative care for patients in the community and at our sites, working with an Occupational Therapist and the wider MDT.

This 37.5-hour week role offers a salary of £29,202 per annum (WTE) with cross-site working, and provides strong training, peer support and a values-driven environment focused on improving quality of life.

Qualifications

• Level 3 qualification in Health or Social Care or equivalent.
• Understanding of palliative care.
• Commitment to person-centred, high-quality rehabilitative palliative care for patients, relatives and carers.
• Experience in working as part of a multi-disciplinary team.
• Strong communication skills.
• Good IT skills across platforms.
• Self-awareness and resilience.
• Ability to work with groups of patients and carers.

Responsibilities

• Support high-quality rehabilitative palliative care for patients in the community and at sites.
• Collaborate with an Occupational Therapist and the MDT to enable patients to manage their condition.
• Work primarily in patients' homes with some attendance at Phyllis Tuckwell sites.
• Contribute to activities that promote quality of life according to patient preferences.
