Job summary

Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of adult social care through timely initial assessments for a London Borough of Tower Hamlets client. You will work with service users in the community for up to six weeks on tailored rehabilitation programmes, supported by in-house Reablement Officers delivering care under your direction.

The role requires HCPC registration, a minimum of two years' post-qualification experience in case work and physical

Qualifications

• HCPC registered Occupational Therapist.
• Minimum 2 years' post-qualification experience (Band 6 level or equivalent).
• Experience in case work and physical rehabilitation in a busy environment.
• London-based for urgent case management.

Responsibilities

• Manage varied caseloads of hospital discharges and community referrals.
• Complete risk-assessed home visits and provide clinical oversight for reablement plans.
• Direct and support Reablement Officers delivering care programmes.
• Work collaboratively with a multidisciplinary team to achieve positive outcomes.
