Job summary

Liquid Personnel are recruiting an experienced Occupational Therapist to join the Reablement Team within Brent Adult Social Care. The role focuses on community-based practice to help residents regain independence after hospital discharge.

You'll deliver strengths-based assessments and interventions, work with a multidisciplinary team, and visit homes across the borough to support functional independence and reduce ongoing support needs.

Qualifications

• HCPC registration is required.
• Experience in community, social care or reablement settings.
• Experience completing functional assessments and recommending equipment/adaptations.
• Ability to manage a caseload effectively.
• Strong communication and MDT working skills.
• Person-centred and strengths-based approach.
• Confidence working independently within the community.

Responsibilities

• Complete strengths-based Occupational Therapy assessments
• Support residents following hospital discharge and community referrals
• Carry out home visits across the borough
• Assess functional needs and identify barriers to independence
• Recommend appropriate equipment and adaptations
• Develop person-centred intervention plans
• Work collaboratively with Social Workers, Care Assessors, Reablement Enablers, Health professionals, Hospital teams
• Promote independence through rehabilitation and goal-focused support
• Maintain accurate records and contribute to effective service delivery
