Qualifications

• Experience in Rehabilitation and Reablement within Local Authority services.
• Experience working in local authority settings.
• Strong communication and organisational skills.
• Experience with adult community rehabilitation.

Responsibilities

• Support the delivery of rehabilitation and reablement programmes for adults living in the community.
• Work alongside Reablement Officers, Physiotherapists, Occupational Therapists, and other health and social care professionals.
• Assist with assessments, reviews, and outcome monitoring.
• Support service users to maximise independence and achieve agreed rehabilitation goals.
• Assist with equipment provision and assistive technology interventions.
• Maintain accurate records using electronic care management systems.
• Collaborate with health, social care, and third-sector partners to support ongoing care needs.
