Job summary

Liquid Personnel is looking for an experienced Occupational Therapist to support hospital discharges and recovery in Dorset. This role involves community visits and a hybrid work model, collaborating closely with health services to improve individual care outcomes.

The successful candidate will provide therapy assessments and interventions, maintain records, and help in hospital discharge planning. The position requires a relevant degree and at least 3 years of UK experience.

Qualifications

• Enhanced Adults DBS (can be obtained if not already held).
• Eligibility to work in the UK.
• SWE (HCPC) Registration.
• Minimum 3 years post-qualified UK experience.
• Access to a car and willingness to travel.

Responsibilities

• Provide occupational therapy assessments and interventions.
• Work collaboratively with health and social care partners.
• Contribute to hospital discharge planning and reablement packages.
• Maintain accurate records and ensure compliance with standards.
• Support the wider Home First Social Care Team.
