Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Band 5 Community Occupational Therapist for its Integrated Community Team. You will join a friendly, supportive multidisciplinary group delivering person-centred rehabilitation in patients' homes and local communities.

You will assess and treat adults with diverse health needs, providing rehabilitation, falls prevention, moving and handling assessments, postural management, equipment provision and environmental adaptations

Qualifications

• Autonomous practitioner with ability to maintain own records.
• Experience in community-based rehabilitation should be demonstrated.
• Ability to work within a multidisciplinary team and contribute to education of junior staff.

Responsibilities

• To provide specialist therapy skills within the Community Care Team.
• To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.
• To assist senior staff in the education and supervision of junior colleagues, assistants and students.
• To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.
• To assist senior staff in development and maintenance of an efficient and effective therapy service.
• To contribute to the inter-disciplinary working within the Community therapy team and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.

We are currently recruiting for an enthusiastic & motivated Band 5 Occupational Therapist to join our Integrated Community Team. You'll be joining a friendly and supportive team known for its collaborative approach and commitment to delivering high-quality, person-centred rehabilitation and enabling individuals to achieve their goals within their own homes and communities.

As a Community Occupational Therapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You will provide holistic assessment and intervention, including rehabilitation, falls prevention, moving and handling assessments, postural management, equipment provision, and environmental adaptations to promote independence, safety, and wellbeing.

You will manage a varied caseload and work closely with colleagues across health and social care to support people to remain at home, prevent avoidable deterioration, and facilitate recovery following illness or hospital admission.
