Job summary

Wight Healthcare is seeking a Band 5 Occupational Therapist to join our Integrated Community Team. You will assess, treat and support adults in their homes and communities, providing holistic rehabilitation and equipment adaptations to promote independence and safety.

You will work within a multidisciplinary team to deliver person-centred care, falls prevention, postural management, and home adaptations, while supporting service development and teaching junior colleagues.

Qualifications

• Band 5 Occupational Therapist with community experience.
• Strong communication and organisational skills.
• Interest in service development and mentoring junior staff.

Job description

We are currently recruiting for an enthusiastic & motivated Band 5 Occupational Therapist to join our Integrated Community Team. You'll be joining a friendly and supportive team known for its collaborative approach and commitment to delivering high-quality, person-centred rehabilitation and enabling individuals to achieve their goals within their own homes and communities.

As a Community Occupational Therapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You will provide holistic assessment and intervention, including rehabilitation, falls prevention, moving and handling assessments, postural management, equipment provision, and environmental adaptations to promote independence, safety, and wellbeing.

You will manage a varied caseload and work closely with colleagues across health and social care to support people to remain at home, prevent avoidable deterioration, and facilitate recovery following illness or hospital admission.
