Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Band 5 Physiotherapist to join the Integrated Community Team. The role offers home-based and community rehabilitation with supervision and preceptorship support for those transitioning into community practice.

You will assess and treat adults, provide mobility and balance rehabilitation, and work with a multidisciplinary team to help patients maintain independence and quality of life.

Qualifications

• Degree or Diploma in Physiotherapy, state registered.
• Current UK professional registration.
• Experience in community/home-based rehabilitation is advantageous.

Job description

We are currently recruiting for an enthusiastic & motivated Band 5 Physiotherapist to join our Integrated Community Team. We provide a supportive & dynamic service, delivering high-quality, person-centred rehabilitation in patients' homes and community settings.

This is an excellent opportunity for a newly qualified or experienced Physiotherapist who is keen to develop their clinical skills, can demonstrate excellent communication & organisational abilities, and enjoys working as part of a team.

As a Community Physiotherapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You'll provide assessment & rehabilitation interventions, including mobility assessment, strength and balance rehabilitation, exercise prescription, falls prevention, and functional recovery, helping patients to maximise their independence, and quality of life.

You'll manage a varied caseload with support from experienced colleagues and work closely with health & social care professionals to support people to remain at home, prevent avoidable deterioration, and recover following illness or hospital admission. You will also benefit from structured supervision, preceptorship support, and a range of learning opportunities designed to support your transition into community practice. Work as part of the multi-professional Locality Health Team delivering programmes of rehabilitation within the community in close collaboration with other relevant services/agencies e.g. other community and inpatient teams, social care, primary and secondary care.

Manage a caseload with supervision as required/be responsible for a defined area of service, undertaking comprehensive assessment, planning and implementation of comprehensive programmes of care for patients who require intensive support / rehabilitation guided by more senior clinicians where required.

Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment to rehabilitation assistants and students on placement.

Work collaboratively with partners in health and social care, to establish a whole systems approach to managing patients with complex care needs, maintaining a "community pull" on patients being admitted to secondary care.
