HCRG Care Group is seeking a focused and caring Community Healthcare Assistant for Malmesbury Primary Care Centre. This part-time role (22.5 hours/week) involves delivering high-quality care in community settings, supporting patients at home, and working closely with a multidisciplinary team to promote independence and wellbeing.

You will follow care plans, maintain records, and travel across the locality as required. NVQ Level 2/3 in Health and Social Care or equivalent is desirable.

Aufgaben

• Deliver high-quality care and support to patients in community settings.
• Assist with personal care and activities of daily living where required.
• Support patients with rehabilitation and promoting independence.
• Monitor and report changes in patients' health and wellbeing.
• Work closely with nurses, therapists, GPs and other healthcare professionals.
• Maintain accurate patient records and documentation.
• Follow care plans, policies, procedures and clinical protocols.
• Promote dignity, respect and person-centred care at all times.
• Support patients and families to achieve positive health outcomes.
