Job summary

Hertfordshire Community NHS Trust is seeking a Community Healthcare Assistant (Band 3) based in Hoddesdon to provide care to patients in their own homes as part of an Integrated Community Team. The role requires some prior caring experience, with NVQ 3 desirable, and you will work across a team of nurses, therapists and rehabilitation staff.

Applicants must be able to work autonomously, manage a varied workload and participate in shift patterns including weekends and evenings.

Qualifications

• Past experience in the caring profession is essential (NVQ 3 desirable).
• Band 3 Healthcare Assistant role within a community team.

Responsibilities

• To work within a team that provides an assessment and care service to patients with nursing/ health needs.
• Visit patients in their own homes and provide appropriate care and support as defined in the Care Plan.
• Undertake administration and clerical tasks related to the care process including record keeping and use of computer.
• Support rehabilitation and promote independent living in patients and, where appropriate, support carers.
