Job summary

HCRG Care Group in Bath is seeking a Community Nurse Band 5 to deliver high-quality home-based nursing within The Hollies team. You will work with GPs and multidisciplinary colleagues to assess needs, plan care, and promote independence in patients’ own homes.

This role requires NMC registration, a UK driving licence with access to a car, and proficiency with IT and MS Office. The position offers AfC terms, NHS Pension, and professional development through our Learning Enterprise.

Qualifications

• NMC registered nurse (1st level) with current pin.
• Full UK driving licence with access to a car for work.
• Competent with IT systems and Microsoft Office packages (Outlook, Word, Excel).

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients and carers.
• Monitor patient progress and escalate changes as needed.
• Prioritise and manage a caseload.
• Maintain accurate patient records in electronic and paper systems.
• Deliver evidence-based care and participate in quality improvement and audits.
• Support student and colleague development.
• Contribute to a safe environment through infection control and risk assessment.
• Manage clinical equipment and stock.
