Job summary

Bradford District Care NHS Foundation Trust is seeking an experienced Registered Learning Disability Nurse to join our Intensive Support Team within the Learning Disability Community MDT. The post requires community-based work with adults with learning disabilities, ideally in crisis situations, and evidence of working in multidisciplinary settings.

Qualifications

• Registered Learning Disability Nurse with NMC registration.
• Significant post-registration experience in learning disability/community settings.
• Experience in assessments and collaboration with multi-disciplinary teams.

Responsibilities

• Provide nursing care for adults with learning disabilities in the community.
• Lead health screening and health facilitation for service users.
• Hold an independent caseload and act as CPA Coordinator.
• Collaborate with the MDT to prevent hospital admission under the Mental Health Act.
