Torbay and South Devon NHS Foundation Trust is seeking a Community Registered Nurse to join the Intermediate Care team across Torquay, Paignton and Brixham. The role supports patients to regain independence and maintain comfort in their own homes.

The post is NHS AfC Band 5, permanent, 37.5 hours per week with weekend rota in a 7-day service. Salary ranges £32,073–£39,043 per annum pro rata; closing date 10/11/2026.

Formación

• Qualified Registered Nurse Level 1 Adult Nursing and active registration.
• Experience in community/intermediate care settings preferred.
• Desire to undertake V150 prescribing and ongoing professional development.
• Diabetes and Palliative care knowledge beneficial.
• Experience with COPD/Heart Failure and health care assessments.

Responsabilidades

• Work as part of the intermediate care team supporting housebound patients in Torquay, Paignton and Brixham.
• Deliver high-quality, patient-centred care and enable rehabilitation and independence.
• Communicate effectively with GPs, primary/secondary care teams and carers.
• Support patients recently discharged from hospital or with declining function.
• Contribute to achieving agreed standards of care and participate in team reviews.
