Torbay and South Devon NHS Foundation Trust invites applications for a role within the Rapid Response Team, covering Torbay and South Devon. You will work as part of a friendly, dedicated team, providing short term care at patients' homes to support independence after illness or hospital stay.

The role requires flexibility, driving and the ability to work across seven days, including evenings. Full UK driving licence is essential; salary ranges £25,760 to £27,476 pro rata, band 3, with Agenda

Formación

• Experience of personal care and/or clinical procedures to NVQ2 or equivalent knowledge.
• Basic IT skills: Word, Outlook, Excel.

Responsabilidades

• Provide personal care and monitor the patient's condition.
• Enable individuals to remain in the community and be supported at home.
• Carry out personal tasks not the responsibility of a nurse.
• Collaborate with other workers and professionals to provide care.
• Promote independence and safety in the home.
