Job summary

NHS Jobs in Ilfracombe and Braunton is recruiting a Band 5 nurse to join our Community Nursing Team for a 12-month fixed term contract. You will be delivering evidence-based nursing care to patients in their home environments, assessing deterioration and intervening to prevent further risk.

The successful candidate will work both autonomously and as part of a team, with a Monday–Friday 9am–5pm pattern and roughly 1 weekend in 4; the post is 22.5 hours per week, fixed term until 29/11/2027.

Responsibilities

• Assess patients and deliver evidence-based nursing care at patients' homes.
• Recognise deterioration and intervene promptly to optimise safety.
• Communicate effectively with patients, families and other professionals.
