Job summary

Midlands Partnership NHS Foundation Trust is recruiting a Band 5 Community Staff Nurse to join the Community Intervention Team in the South West Staffordshire locality, based at Greyfriars Therapy Centre, Stafford. You will play a key role in enabling patients to remain in their own homes and avoid unnecessary hospital admissions, while delivering safe, effective and responsive care.

Qualifications

Degree in nursing or equivalent
Registration with NMC
Mentorship qualification (998-997)

Responsibilities

Be a Registered Adult Nurse with current NMC registration.
Hold a Degree in Nursing or equivalent qualification.
Have experience working within community nursing or acute care.
Demonstrate excellent communication and interpersonal skills, with the ability to build effective relationships with patients, carers, and multidisciplinary teams.
Possess clinical assessment, problem-solving, and decision-making skills.
Be committed to delivering safe, effective, and compassionate patient care.
Show a commitment to continuous professional development and lifelong learning.
