Job summary

Bradford District Care NHS Foundation Trust is seeking motivated Band 5 Community Nurses to join our locality teams and deliver high quality person-centred care to people in their own homes across Bradford.

You will work a seven-day rota, enabling flexible hours, with a structured preceptorship for new nurses and opportunities to develop skills in tissue viability and palliative care, supported by agile technology and a supportive trust environment.

Qualifications

• Qualified nurse with active NMC registration and RGN status.
• Willingness to undertake education/training within the service.
• Must be able to travel across the district and have a full UK driving license.

Responsibilities

• Deliver high quality person-centred nursing care to patients in their own homes.
• Collaborate with the Primary Health Care team and local specialists for care planning.
• Work on a seven-day rota with flexible hours to cover home visits.
