Job summary

Bradford District Care NHS Foundation Trust is seeking a Band 5 district nurse to join the home visiting service within Bradford. You will provide hands-on nursing care to patients in their homes, working as part of the district nursing team and collaborating with primary care colleagues.

The role supports patients with long-term conditions, complex needs and palliative care, with a structured preceptorship for new entrants and flexible working arrangements.

Qualifications

• RGN with active NMC registration.
• Understanding and/or experience of interagency working and adult protection.
• Willingness to undergo education and training for practice and service need.
• Demonstrates ongoing professional development.
• Able to travel between multiple sites to undertake numerous visits.

Responsibilities

• Deliver high quality person-centred nursing care to people in their own homes and/or clinic settings.
• Work as part of the district nursing team.
• Assess health needs and deliver care to the practice population, working with the wider primary health care team.
• Provide care for people with long-term conditions, complex needs and palliative care.
• Work rotational shifts across a seven-day service, between 8am and 9pm, to cover a 24-hour service.
• Use agile technology to deliver timely care, with support from Trust specialists.
• Travel extensively across the district to make visits.

Job description

This Band 5 post sits within the district nursing team at Bradford District Care NHS Foundation Trust, with vacancies across several community locality teams. The work is hands-on nursing care in patients' own homes, covering long-term conditions through to complex and palliative needs, alongside primary care colleagues. The service runs a rotating seven-day rota with shifts between 8am and 9pm, and it suits both newly qualified and experienced nurses, with a structured preceptorship. Applicants need NMC registration and a full UK driving licence with access to a car.
