Job summary

Bradford District Care NHS Foundation Trust is seeking motivated Band 5 Community Nurses to join locality teams. Qualifications

• Registered General Nurse with current NMC registration.
• Experience in community/district nursing or equivalent.
• Full UK driving licence and access to a car is essential.

Responsibilities

• Deliver high-quality health care to patients in their own homes and clinics.
• Collaborate with the wider Primary Health Care team and participate in health needs assessments.
• Work on a 7-day rota to provide a 24-hour service with flexible hours.
• Support patients to remain independent, safe, and well in the community.
