Job summary

East Suffolk and North Essex NHS Foundation Trust is seeking a Band 5 Community Nurse to join the North West Integrated Neighbour Team, delivering holistic nursing care in patients' homes across rural and urban areas. You will work within a supportive MDT and develop your clinical skills.

You must hold RN Part 1, be registered with the NMC, and have IV administration experience. A valid driving licence and car are essential for community-based work.

Qualifications

• RN Qualification (Part 1 of register).
• Current NMC registration.
• IV Administration / cannulation.
• ENB 998/mentorship preparation or equivalent.

Responsibilities

• Provide holistic nursing care in patients' homes as part of the INT team.
• Collaborate with Community and District Nurses and MDT members to coordinate care.
• Commit to ongoing professional development and participate in training and forums.
• Maintain accurate patient records and document care plans.
