Job summary

West London NHS Trust is seeking a Band 5 Community Staff Nurse to provide in-home care, support discharge planning and work within a multidisciplinary team. The post focuses on delivering person-centred care in patients’ homes, with integration alongside GPs, ambulance services and social services.

The role requires 12 months post-registration experience, community nursing exposure and commitment to ongoing professional development within a 7-day service model.

Qualifications

• Recent post-registration nursing role experience is required.
• Experience in community settings is desirable.
• Ability to supervise others and work in a multidisciplinary team.
• Knowledge of professional standards, governance and safeguarding.

Responsibilities

• Assess, plan, implement and evaluate nursing care for patients in homes.
• Support admission avoidance and safe discharge from hospital.
• Collaborate with GPs, ambulance service, social services and community partners.
• Demonstrate strong communication, organization and time-management.
