Job summary

HCRG Care Group invites applications for a Band 5 Community Nurse based around Sheppey, covering Sheppey, Sittingbourne and nearby villages. You will provide compassionate, patient-centered nursing care in patients' homes and local communities, assessing needs, planning and evaluating care with a multidisciplinary team.

Ideal candidates are registered nurses with valid NMC registration, a driving licence, and a desire to support patients in managing long-term conditions and avoiding hospital

Qualifications

• Registered Nurse with a valid NMC registration.
• Full UK driving licence and access to a vehicle for work purposes.
• Experience or interest in community/district nursing is desirable.

Responsibilities

• Deliver high-quality nursing care in patients' homes and community settings.
• Assess, plan, implement and evaluate care for individuals and families.
• Work with GPs, therapists and social care teams to coordinate care.
• Maintain accurate clinical documentation and contribute to service improvements.
• Promote independence and wellbeing while managing a varied caseload.
