Job summary

Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Community Staff Nurse for the South Shore area on a temporary fixed-term basis at 30 hours per week. A full UK driving licence and car access are essential to cover Fylde coast locations, with NMC Level 1 registration and enhanced DBS clearance required.

You will provide safe, patient-centred care in homes or clinics, plan, implement and evaluate care, and escalate as needed to the senior community nurse.

Qualifications

• NMC registration (Adult branch) and Enhanced DBS clearance are required.
• Full UK driving licence and access to a car are essential.
• Community nursing duties in patients' homes or clinic settings.

Responsibilities

• Provide safe, effective, individualised care to patients.
• Plan, implement and evaluate programmes of care; escalate when required to the senior community nurse.
• Monitor patients' health needs in partnership with families and carers; refer for reassessment when required.
• Work across the Fylde coast in patients' homes or clinic settings.
