Job summary

Sussex Community NHS Foundation Trust is inviting applications for a Registered Nurse to join the Adur PCN Team in Lancing. The role covers housebound patients in the Adur locality, offering wound care, long-term condition management, and palliative care in patients’ homes and care home settings.

Qualifications

• RN qualification or degree or diploma.
• ENB 998 or equivalent or working towards.
• Evidence of a range of clinical competencies.

Responsibilities

• Assess, plan, deliver and evaluate patient care in the community.
• Wound assessment and management.
• Catheter and continence care.
• Medication administration, including insulin where competent.
• Support patients with long-term conditions and complex health needs.
• Deliver palliative and end-of-life care and safeguarding.
• Collaborate with GPs, therapists, care homes and the multidisciplinary team.
• Support hospital discharge and prevent unnecessary admissions.
• Maintain accurate clinical records and contribute to care planning.
