Job summary

Sussex Community NHS Foundation Trust is recruiting a Band 5 Registered Nurse to join the Adur PCN Team in Lancing, delivering high‑quality care to housebound patients in their own homes and care home settings.

You will work as part of a friendly multidisciplinary team, with a structured induction, ongoing support and development opportunities to build your community nursing skills in wound care, long‑term conditions, palliative and catheter care, safeguarding and hospital discharge support.

Qualifications

• Registered Nurse with current NMC registration.
• Wound management and catheter care experience are desirable.
• Ability to manage long-term conditions and provide safeguarding.

Responsibilities

• Assessment, planning, delivery and evaluation of patient care.

The Adur Community Primary Care Network Teamis looking to recruit a Registered Nurse to join our friendly and supportive team based in Lancing, providing care to housebound patients across the Adur locality. Whether you are newly qualified or an experienced nurse seeking a new challenge, Community Nursing offers a rewarding and varied role, including wound care, long-term condition management, palliative care, catheter care, safeguarding, and care home support. As a Band 5 Community Nurse within the Adur PCN Team, you will provide high-quality nursing care to housebound patients in their own homes and care home settings.

The main aspects of the role are Diabetic Management, Wound Management including Leg Ulcers diagnosis and treatment, Palliative care working very closely with the Macmillan and GP's, Bladder and Bowel care including urethral and supra pubic catheters and other continence needs. As well as the traditional type of patients as stated above, we work in partnership with the local health and social providers covering a specific group of people under Frailty. Our goal is to deliver high quality care in the community, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.
