Job summary

Qualifications
- Must be a registered nurse with active NMC pin.
- Experience in community nursing and care planning preferred.
- Ability to deliver patient-centred care in a home setting.
Responsibilities
- Provide adult community nursing care to patients in East Grinstead and Crawley Down areas.
- Manage diabetes and wound care, including leg ulcers and catheters as required.
- Support rehabilitation, self-management and palliative care in the community.
