Job summary

HCRG Care Group is seeking a Community Nurse (Band 5) to join the Melksham team, delivering high-quality, patient-centred care in patients' homes. You will work with GPs and multidisciplinary teams to assess needs, plan and deliver care, promoting independence and positive health outcomes.

You will be NMC registered, possess a full UK driving licence and be proficient with IT systems. The role offers 37.5 hours per week at Melksham Community Hospital with AFC terms and NHS Pension.

Qualifications

• NMC/1st level registered nurse required.
• Full UK driving licence with access to a car for work purposes.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients and carers.
• Monitor progress and escalate changes in condition appropriately.
• Maintain accurate patient records using electronic and paper systems.
• Deliver evidence-based care and contribute to quality improvement and clinical audits.
• Promote health, wellbeing, and independence for patients and carers.
• Participate in team meetings, handovers, supervision, and mandatory training.
• Support learning and development of students and colleagues.
• Ensure safe environment through risk assessment and infection control standards.
