Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Community Staff Nurse to join the West Berkshire Community Nursing team. You will assess, plan, and deliver care in patients' homes, including complex wound care and palliative support, working with families and carers.

The role covers a seven-day service with flexible working options and opportunities for training, the NHS pension, and ongoing professional development.

Qualifications

• Registered General Nurse (RGN).
• Evidence of continuing professional development.

Responsibilities

• Assess patient and carer needs holistically, and plan, implement and evaluate nursing care in partnership with patients and families
• Carry out first visits, take immediate action and report back to the team leader, Band 5 or community matron
• Build working relationships with other professionals and voluntary agencies to achieve health aims
• Undertake Practice Assessor and Supervisor training and support students and new staff
• Deliver care in line with evidence-based practice and help develop that practice
• Promote self-care and build health promotion into patient care
• Develop and deliver teaching programmes for patients
• Demonstrate skills in assessment, medicines management, bladder and bowel care, tissue viability, specialist equipment, palliative care, symptom control and reducing admissions, to the Band 5 competency framework
• Provide care for people with long-term conditions, carrying out reviews against individual care plans
