Qualifications
- Must be a registered practitioner to degree/diploma level.
- Membership of the relevant professional body is required.
- Evidence of recent professional development in an up-to-date portfolio.
Responsibilities
- Provide high-quality, person-centred care in the home setting.
- Assess patient care needs and develop care programmes with the multi-disciplinary team.
- Manage acute and chronic nursing needs and build therapeutic relationships with patients.
- Identify care needs, monitor conditions, record observations and support wellbeing plans.
- Support patients to maintain wellbeing and independence and provide end-of-life care when needed.
- DBS check will be required and the cost will be covered by the Trust.
