Qualifications

• NMC/1st level registered Nurse status.

Responsibilities

• Participate in assessment and care planning with the Team Manager and specialist team support.
• Implement and evaluate care per the care plan; escalate changes to the Team Manager/GP.
• Communicate effectively with patients, carers and the multidisciplinary team.
• Prioritise daily work according to clinical and patient need.
• Maintain accurate patient records in line with professional requirements.
• Implement evidence-based care and best practice.
• Actively participate in team meetings and daily handover.
• Initiate health promotion activities with individuals, patients and carers.
• Contribute to development and implementation of community nursing service developments.
