Job summary
The Hayle-based Community Nursing Team is seeking a caring and motivated Band 5 Community Nurse to join a large, busy NHS team. You will provide patient-centred nursing care at home across Hayle, St Ives, Marazion and surrounding areas, collaborating with GP practices and social care colleagues to maintain high standards of care and ongoing professional development.
They work a two-shift model including 8am–10pm coverage with core shifts 08:30–16:30 and 14:00–22:00, with weekend work.
Qualifications
- Registered Nurse (Adult) with current NMC registration.
Responsibilities
- Assessment, planning, implementation and evaluation of care in patients' homes.
- Work with patients and carers to support informed choice.
- Support the Community Nurse Team leader.
- Work autonomously with evidence-based care in line with policies.
- Move flexibly to support teams as service demands.
