Job summary

Surrey Downs Health & Care is seeking Band 5 Nurses to work at East Elmbridge PCN within SDHC. If you are passionate about caring for people and making a difference in the community, we would love to hear from you!

We welcome nursing staff from all backgrounds, including newly qualified. You will assess care needs, develop and evaluate holistic care plans, and contribute to team management. A current UK driving licence and access to a vehicle are required.

Qualifications

• Registered nurse with current UK registration
• Band 5 nurse with valid driving licence (or willing to obtain)
• Ability to work in community settings and travel between sites

Responsibilities

• Assess care needs and develop holistic care plans for individuals in the community
• Implement and evaluate care packages in collaboration with the patient and family
• Provide mentoring and supervision to team members as required
• Maintain accurate, timely records and data entry in line with SDHC procedures
• Collaborate with other professionals and agencies to ensure safe, effective care

We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.
To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.
Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.
We have our own Learning & Development Department that have a range of different courses and development plans that give you the opportunity to grow your career.

• Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
• Treat all patients as individuals, respecting their privacy and dignity at all times
• Assess individual care needs, implement and evaluate holistic programmes of care
• Involving, supporting, informing and educating family/carers
• Take every opportunity to promote health and wellbeing of the patient
• Develop effective communication and liaison systems
• Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures
• Facilitate/undertake training and professional updating for team and students
• Act as an assessor and mentor to qualified and unqualified staff
• Maintain, develop and monitor standards of evidence based nursing care
• Maintain own professional and clinical integrity in line with NMC guidelines
• Support/mentor team members and participate in clinical supervision activities
• Work without direct supervision and to maintain close liaison with the senior team member, deputising as required
• Be responsible and maintain timely patient care
• Be responsible for maintain data entry and other record keeping
• To work in co-operation and partnership with all other professionals and agencies involved in the care of the patient
• Be aware of and act upon when necessary, procedures that are in place to protect vulnerable individuals
