Job summary

Surrey Downs Health & Care in Thames Ditton is seeking a Band 5 Nurse to join East Elmbridge PCN on a fixed-term 11-month contract, 30 hours per week. You will assess and plan holistic care for patients in the community, supporting families and carers.

Applicants should be a Registered Adult Nurse with NMC, hold a valid UK driving licence, and commit to delivering safe, evidence-based care within SDHC guidelines. Weekend availablity on a 1 in 4 rota is required.

Qualifications

• Registered Nurse with NMC registration.
• Valid UK driving licence to drive in the community.

Responsibilities

• Assess individual care needs and develop holistic care plans.
• Deliver and evaluate nursing care within community settings.
• Educate patients, families and carers to promote health and wellbeing.
• Maintain accurate records and liaise with other professionals.
• Undertake leadership and mentoring within the team.
• Work with District/Nurse teams and contribute to governance.
