Job summary

Surrey Downs Health & Care is seeking a Community Neuro Rehab Assistant to support an Integrated Stroke and Neurorehabilitation Service. You will deliver therapy programmes and assist in planning under therapists’ supervision.

You will work closely with physiotherapy and occupational therapy teams, monitor progress, and contribute to patient goals. A UK driving licence and access to a car are essential for community visits.

Qualifications

• Excellent written and verbal communication with patients and carers.
• Compassionate and proactive approach to patient support.
• Ability to work independently and within a multidisciplinary team.
• Driver with valid UK licence and access to a car is essential.

Responsibilities

• Deliver neuro-rehabilitation programmes in outpatient, home, and community settings under therapist guidance.
• Support Physiotherapy and Occupational Therapy interventions.
• Monitor patient progress and contribute to therapy planning within guidelines.
• Carry out basic patient assessments and collect information to adapt treatment plans.
