Job summary

British Red Cross is seeking a Community Navigator to support people in Denbighshire through social prescribing and linking with local services. The role is based on-site across Denbighshire with a fixed term contract until March and a pro rata salary.

You will work with Community Resource Teams and healthcare professionals to assess needs, develop support plans, and connect service users with appropriate community and third sector resources, while maintaining confidentiality and strong

About The Role

Community Navigator Location: Covering county of Denbighshire (On-site) Salary: £, to £, per annum, pro rata Hours: per week Contract: Fixed term contract (until st March ) Additional Requirement: Applicants must have a full licence ( years) and access to a vehicle. Do you have knowledge of services (both statutory and third sector) which are available within Denbighshire community? Then this role could be for you! Our team of Community Navigators in Denbighshire provide a link between Health and Social Care professionals, Denbighshire citizens, their family, and carers. As part of the team, you would provide assistance within the community and third sector. This approach can also be known as ‘Social Prescribing’ which is a mechanism for linking citizens with non-medical sources of support within the community. You will recognise that people are experts in their own life but sometimes they need someone else to help them to identify and work towards the outcomes that they would like to achieve.

What will a day in the life of a Community Navigator involve?
  • You will work closely with the Community Resource Teams, ‘Talking Points’, SpoA Wellbeing Coordinators, Complex Disability Team, Primary care, Community Mental Health Teams and other people from a range of local services.
  • You will engage with service users of Talking Points in their normal place of residence or perhaps at another setting. You will carry out needs assessments and develop support plans utilising the “What Matters” approach.
  • You will be a source of current, accurate and timely information and knowledge about a comprehensive range of help available within the community. You will improve the service quality for the benefit of service users.
What do you need to be a successful Community Navigator?
  • Being proactive, can react quickly to service requests and appreciate the importance of the service you are providing.
  • Dealing with queries in a diplomatic, confidential and non judgemental manner.
  • Comfortable planning your own workload and working remotely whilst still part of a team.
  • Excellent communication skills, both written and verbally.
  • IT literate, particularly with Microsoft packages
