Gloucestershire Health and Care NHS Foundation Trust is seeking an experienced Transformation Programme Lead for an 18-month, full-time fixed-term post. The role drives system-wide improvements across community mental health, 24/7 neighbourhood services and urgent care pathways, partnering with primary care, VCSE and Families First.

Qualifikationen

• Master's degree or equivalent in a related field.
• Improvement qualification such as QSIR Practitioner or QI Gold is required.
• Programme management qualification such as MSP is recognised.

Aufgaben

• Manage the overarching transformation programme, projects and workstreams from set-up to benefits realisation.
• Drive and coordinate transformation activity with system partners and stakeholders across mental health services.
• Develop the core community mental health model, including MDT working and joint working with primary care and VCSE.
• Develop 24/7 neighbourhood community mental health services, including early intervention.
• Develop the urgent and emergency mental health pathway, including an all-age pathway review (MEN-SAT) and a 24/7 urgent and emergency care model.
• Engage in Families First, including the CAMHS front door model and navigation hub pathway.
• Develop and maintain a programme management framework that gives assurance on delivery, with reporting locally, regionally and nationally.
• Apply project management, service improvement and quality improvement methods to keep initiatives coherent.
• Lead and coach team members, including line management of new roles where needed, and manage day-to-day staff matters.
• Analyse complex workforce, financial, demand and capacity data to optimise performance.
• Plan new ways of working and support long-term strategic plans for service improvement across the health and care system.
• Co-produce new service models with service users, stakeholders and partners across the whole pathway.
