Wakefield Council is seeking a Mental Health Support Worker to join the Enhanced Team East in Pontefract to support adults with significant mental health needs in the community and promote recovery, independence and social inclusion. You will help clients engage in social, leisure, educational and occupational activities and contribute to care planning.

The role is full-time (37 hours per week), with 8:30–17:00 hours, Monday to Friday and weekend rota.

Qualifications

• Basic understanding of English and Maths.
• Awareness of types of mental health problems and their impact on lives of individuals and their carers.
• Some knowledge of mental health policy and legislation.

Responsabilités

• Support people in managing their mental health in the community.
• Promote recovery, independence and social inclusion.
• Develop, implement and review care plans for the people we support and their carers.
• Work alongside Social Workers and mental health colleagues from South West Yorkshire Partnership NHS Trust.
• Build and sustain relationships with individuals and families to improve mental health.
• Identify and help people achieve outcomes with a focus on recovery and independence.
• Provide practical and emotional support to clients.
