Devon Partnership Trust is seeking a Community Support Worker to assist adults with mental health needs in the Tiverton area. The role focuses on practical, person-centred support to build daily living skills and independence within the community.

You will work within a multidisciplinary team including nurses, social workers, occupational therapists, psychologists, and psychiatrists, providing outreach, monitoring wellbeing and promoting engagement with treatment plans.

Qualifications

• Demonstrates ability to maintain personal boundaries and communicate effectively.
• NVQ Level 3 / QCF Level 3 in Health and Social Care or equivalent.
• Care Certificate to be completed within first 6 months of employment.

Responsabilités

• Provide practical, person-centred support to help individuals manage daily living tasks.
• Carry out regular outreach visits in the community or at home to promote engagement with care plans.
• Support patients toward positive health and signpost to wider community resources.
• Work with the multidisciplinary team to share observations and escalate concerns as needed.
• Maintain accurate records of support and safeguarding concerns.
• Promote recovery-focused practice and respect individual choices and goals.
