Job summary

Norfolk & Suffolk Foundation NHS Trust in the UK is seeking a Band 6 Community Mental Health Practitioner to join the Waveney Adult Community Mental Health Team. The role requires registration as a Mental Health Nurse, OT, or Social Worker with experience in community settings and working-age adults under the Care Programme Approach.

Qualifications

• Qualified professional Nurse (Mental Health or Learning Disability) or Occupational Therapist or Social Worker with relevant registration.
• Practice Assessor and Practice Supervisor training (NMC registrants) or Practice Educator (AHP & Social Worker Registrants).
• Ongoing professional development toward degree level if registration is upon diploma level.

Responsibilities

• Plan and deliver direct patient care with multidisciplinary and multi-agency involvement.
• Manage a caseload and provide mental health care services to people with mental health care needs.
• Be responsible for accurate mental health clustering on admission, discharge and review.
• Ensure clustering information is shared with other Trust teams on transfer of patients.
• Ensure accuracy and quality of mental health clustering activity within the team.
• Applicants must hold a full UK driving licence.
